The CM policy regarding the use of Medication (5501 and 5502) must be followed for medication to be administered during school hours. It is the preference of the CM School District that all children's medication, prescription or non-prescription, be administered at home. If your child must take a prescribed medication or over the counter medication at school, please have the appropriate medication form(s) completed and returned along with the medication to the school nurse. This form(s) can be obtained from the student handbook, school nurse or download below.

Medication Form

ANY OVER THE COUNTER MEDICATION MUST BE IN THE MANUFACTURER'S PACKAGING AND PRESCRIPTION MEDICATION MUST BE IN THE APPROPRIATE PHARMACY LABELED CONTAINER.

Parents/Guardians are responsible for facilitating the transportation of medication to and from school. Medication should not be transported by the students.
